There will be two mandatory virtual training sessions available for you to attend prior to the big event: 12:00 p.m. on Monday, June 5 or 6:00 p.m. on Tuesday, June 6 that you will be also be able to sign up for after you complete this registration form. You only need to attend ONE of the training sessions. If you are unable to make either of the sessions, email Sara at endoflifeloptionsma@gmail.com and we'll work with you to ensure you have the training you need.
